Instructions for finding a list Workers’ Compensation Physicians and Facilities in your area:

1. Visit www.CompanionGroup.com.
2. Select Network Management
3. At the middle of the screen select Provider Directory
4. On the left-hand side of the screen, select Quick Search
5. You must enter a valid zip code or city and state; select Find Providers
6. At the bottom of the first page: elect to email or print the document

(Please be sure to post the list in a conspicuous place readily accessible to employees at all times.)

Instructions for looking up a specific Physician or Facility:
1. Visit www.CompanionGroup.com
2. Select Network Management
3. At the middle of the screen select Provider Directory
4. Option #1: On the left-hand side of the screen, select Address Search
a. For a quick search, type your city/state or zip code. Select the appropriate: Search Distance, Sorting Results and/or Providers per page and click continue. (Confirm the Network: Companion Networks.)
b. Select the Provider Types and Specialties
c. Click Find Providers and select provider(s). You have the option to Create a Directory, Panel or an Excel File
5. Option #2: On left-hand side of screen, select Name Search
a. Enter Provider/Facility Name and pertinent information; click Find Providers
(Confirm the Network: Companion Networks.)
b. Select the Provider. You have the option to Create a Directory, Panel or an Excel File
6. Option #3: On left-hand side of screen, select Region Search
a. Enter pertinent information for your search. Select the appropriate: Sorting Results and/or Providers per page and click continue. (Confirm the Network: Companion Networks.)
b. Select the Provider Types and Specialties
c. Click Find Providers and select provider(s). You have the option to Create a Directory, Panel or an Excel File

Should any employee have an accident, it is critical that you immediately notify Companion Property & Casualty. This allows our professional nurse case managers and claims adjusters to ensure that your employee receives the most appropriate care and cost-effective medical treatment. Timely reporting also has a favorable influence on the premiums that you pay.

Save Time...Save Money
Immediate notification of claims allows us to properly investigate and evaluate the claim, as well as to effectively manage the medical care that your employee receives. As soon as an injury occurs, send us a "First Report of Injury" form. Please note that submission of the "First Report of Injury" form is not an admission of any type of liability. First Reports of Injury may be submitted as follows:

Call us toll free at: 1-800-845-2724 (menu option 2) - especially if the injury requires emergency treatment or immediate hospitalization.

Submit electronically: CLICK HERE

Submit via fax at: 1-877-379-7389 - using a copy of the "First Report of Injury" form you received with your policy, or by downloading off of our website by clicking on this link Workers Comp Forms, followed by the state selection for the state where the injury occurred.

Via e-mail at: wcclaims@companiongroup.com - using a copy of the "First Report of Injury" form as an attachment

However the claim is reported, you should be sure to do so in the most expedient manner and be sure to send the employee to one of the physicians on the Companion Property & Casualty physician panel.
